What is the PicoSure?

Cynosure’s PicoSure is the FIRST and only Aesthetic Picosecond laser for the safe and effective removal of tattoos and benign pigmented lesions, as well as acne scars, wrinkles and skin revitalisation treatments.

PicoSure is the first of a new generation of aesthetic lasers which don’t rely solely on heat to burn or melt away unwanted tattoo ink or melanin, the pigment which causes dark spots in your skin.  Instead, PicoSure engineers used cutting edge physics and medical science to help practitioners treat patients in a new way!

“Groundbreaking innovation. Unique technology.”

What’s more, clinical data shows better clearance in fewer treatments – with it being PROVEN that it can remove tattoos with 75% less sessions than standard tattoo removal lasers!

Cynosure’s PicoSure laser treats at two wavelengths: 532nm and 755nm making it ideal for treatment of pigmentary conditions and ALL colours of tattoo ink.  The FIRST machine of its kind to prove and substantiate this claim.

With the addition of the Focus™ Lens Array, we are able to treat stubborn skin conditions in addition to successfully removing tattoos.

What makes it so different to other lasers? What’s the technology?

PicoSure is undoubtedly an unprecedented innovation in laser technology when compared to ordinary Nanosecond lasers.

Picosecond vs. Nanosecond Technology

Traditional nanosecond lasers predominately rely on photothermal action, delivering heat to the pigment and surrounding tissue – often resulting in damage to the tissue and surrounding tissue areas which don’t always repair.  The extreme heat from photothermal action can scar or leave behind white ‘ghosting’ in the skin, long after the course of treatment is finished.

PicoSure’s ultra-short pulse duration goes beyond photothermal action creating an intense photomechanical impact, patented PressureWave, resulting in greater disruption of the target (ink or pigment) for better clearance with fewer treatments, less fluence and without injury to the surrounding skin.

  • Picosecond pulse width is 100X shorter than nanosecond technology.
  • Only ultrashort pulse durations trigger photomechanical effect
  • With this short pulse width, half the fluence is required compared to Q-Switched nanosecond lasers.

PressureWave Technology is used to shatter the target ink into tiny particles which are easily eliminated by the body.  Even dark, stubborn blue and green inks, as well as previously-treated, recalcitrant tattoos can be removed.

In addition, the PicoSure boasts a ‘Boost Adjustable Pressure’ feature which amplifies PicoSure’s PressureWave™ to levels never before possible. Magnifying pressure 70% and shortening pulse widths to 550ps, allows for unprecedented treatment options.

Boost is ideal for recalcitrant tattoos and enhanced shattering of pigment for optimised results.

In short…

“’Picosecond’ is an unparalleled breakthrough in laser technology and delivers ultra-short pulse bursts of energy to the skin in trillionths of a second.  Picosecond pulse width is 100 times shorter than nanosecond technology, enabling unmatched photomechanical impact for better clearance with fewer treatments and less fluency – sparing the skin high thermal damage and targeting the ink/pigment chromophore with precision not found in ordinary nanosecond lasers”

The FUTURE is here!! The FUTURE is only available in Wales and the South West @SkinDeep!

What can PicoSure be used to treat?

Picosure can effectively and safely treat a number of skin concerns and is used all over the world by renowned Plastic Surgeons and Dermatologists in the Cosmetic and Aesthetic Industry.

The range of Treatment Protocols it is used for include..

1) Tattoo Removal

PicoSure is unprecedented when it comes to Tattoo removal as it is the only machine in the world which can remove ALL colours with no skin damage and in over 95% of cases TOTAL CLEARANCE.

 – The 755nm wavelength is able to target black, blue, and green tattoo inks, while the optional 532nm wavelength offers effective treatment of “sunset colours”—red, yellow and orange—which are typically hard to correct.

  • – treatments can additionally be tailored, using variable spot sizes
  • – boost mode can be used to better treat recalcitrant tattoos with shortened pulse width.

2) Pigmentation/Pigmented Lesions.

PicoSure also optimally targets melanin to treat a range of pigmentary conditions. The unique advantage of this is that the pigmented skin incurs minimal thermal damage due to the picosecond pulse duration.

Pigmentation affects so many people.  Although, some types of pigmented lesions exist at birth, more often pigment represents one of the first signs of ageing.  That is why so many people are interested in removing pigment.  In fact, this market is about double the size of the market for the removal of tattoos.

How does the PicoSure remove Pigmentation?

In the same way it removes tattoo ink!

Using the principle of selective photothermolysis, light-based treatment targets different chromophores in the skin.

Chromophores (the pigment) selectively absorb the laser or light energy as heat and yield the desired response – to break them down into tiny pieces the body can absorb and dispose of.

These optimal wavelengths of light clear pigmented lesions by targeting melanin. The damaged pigmented lesions are then cleared by the body.  They undergo a mild desquamation and slough off, removing the appearance of the lesion from the skin.

  • Additionally all treatments are entirely customisable with 2-6mm, 8mm, and 10mm spot sizes and have proven clinical validation, with 26 publications and 59 abstracts to date.

3)  Skin Revitalisation – reduction in acne scarring, wrinkles and additional signs of ageing

With the addition of the Focus Lens Array, the PicoSure has also become a revolution in Skin Revitalisation.

Cynosure’s Focus Lens Array creates microscopic, photomechanical reactions in the epidermis which communicate with the dermis, leading to increased collagen and elastin production  in the skin.  This breakthrough enables skin revitalisation with virtually no downtime—the kind only seen with our PicoSure laser.

This can be used to reduce acne scarring and wrinkles and revitalise the skin in a way performed by no other laser on the market!

Picosure – Skin Revitalisation

The simple fact is that our bodies already have what it takes to help us look and feel younger.

Let PicoSure put it to work for you.

Recent discoveries have begun to reveal the secret language of the body and how cells talk to each other to keep us healthy. Known as cell signalling, this biological communication occurs when cells are injured and release proteins, which are then detected by surrounding cells to trigger a healing response.

PicoSure’s PressureWave™ technology uses a unique wavelength of light (755nm) and a specialized lens to convert laser energy into gentle pressure. This pressure squeezes cells, activating the natural cell signalling processes that create new collagen and elastin without burning or damaging the skin.

This specialised Lens is known as the Focus Lens Array

What is the Focus Lens Array – FLA?

The PicoSure FOCUS Lens Array is a special attachment that is added to the Picosure system to allow it to quickly and painlessly treat common skin concerns gently and effectively.

The Focus Lens Array has a special ‘honey comb lens’ in design, meaning that as the skin is treated, with each pulse, the Focus Lens is reaching inside the skin’s deepest layers.

More importantly however, when it does so, the lens is actually only focusing in on 10% of the treated area with high-energy pulses, while the remaining 90% of the skin is pulsed with low-level energy.

This is key because it allows for a deeper energy infiltration to the dermis below the skin- simultaneously creating elastin and collagen growth whilst promoting a non-traumatic healing effect.

The Process

– The Focus Lens Array delivers 20x the energy density of a normal PicoSure pulse to begin activating cell signalling..

– Intra-epidermal Laser Induced Optical Breakdowns (LIOBs) form and cause pressure waves that propagate into the dermis, triggering pressure injuries to activate cells.

– Cell signalling initiates an enhanced inflammatory healing response that leads to increased collagen and elastin and ultimately revitalizes skin.

This has the result of softening acne scars and stretch marks and reducing the appearance of wrinkles. This process promotes better and faster results, shorter downtime and is safer than other methods.

What can it be used to treat?

PicoSure with the Focus Lens Array is the only FDA approved laser for

  • Wrinkles
  • Acne scarring
  • Hyper-pigmentation/freckles/age-spots

In truth, it is a great alternative for patients who are looking for something a little stronger than chemical peels, micro-needling or microdermabrasion but can in truth be used in combination with these other modalities to achieve optimum results well beyond Client expectations!

Who is a suitable candidate?

The FLA can be used on any skin type and is suitable for anyone with mild to moderate wrinkles, fine lines, acne scars, stretch marks and uneven skin texture. It quickly restores smooth skin and contours irregularities.

The ideal patient should be in good overall health, understand the treatment risks and benefits and have realistic treatment goals. Patients should not be pregnant or nursing or be using photosensitive medications.

Is there any downtime? Does it hurt?

The FOCUS Lens Array uses a microlens (also used for tattoo removal) that has a honeycomb design that sections off into small hot sections that allow for faster, safe treatment with no pain.

However, it is a laser – so heat is induced. Therefore after the treatment some patients experience mild redness which disperses within a few hours.

Clients can generally expect some redness (goes away within one hour or so) and swelling (sometimes the response you’re looking for but very mild). Both of these are moderate and disappear as quickly as they appear.

It is certainly fair to say there are less chances of side effects than similar treatments carried out with fractional and ablative lasers.

What Results can I reasonably expect?

If being treated for skin revitalisation and wrinkle reduction, Clients can reasonably expect to achieve a more youthful glow and healthier looking skin. The Focus Lens Array stimulates the production of collagen – so wrinkles may appear reduced and skin tone will improve in texture and tone.

Elements of restoration of lost volume should be apparent and the skin will appear tighter.

How many sessions will I need?

Every Client is different and depending on a number of factors including skin condition, immune system, genetics, age and whilst considering all of the other combined complimentary treatments we can offer you at our Clinic, we will create a personalised treatment plan specially for you!

For the best results, we typically recommend a series of 3-5 treatments conducted 2 – 4 weeks apart.

The Quanta Q-Plus Series Laser

At Skin Deep we continually invest in our Clinic and pride ourselves on offering CHOICES when it comes to any kind of treatment – Laser or otherwise!

We do not believe that any one product or technology is solely the best at what it does… one size does not ‘fit all’ and some benefits of some technologies can easily compliment or enhance the effectiveness of others.,. which is why we have introduced the Quanta Q-Plus A laser to our portfolio.

The Quanta Q-Plus range uses advanced second generation technology for unrivalled results on solid black tattoos and a huge spectrum of pigmented lesions.

The technology used in this machine uses a ‘Photo-acoustic’ effect of the Q-switched laser beam, for high peaked power which shatters solid targets such as dense black tattoo ink particles and melanosomes in pigmentation.

The advancements used in Quanta Q-Plus q-switched sources can generate a photo-acoustic effect targeted at inks and pigmentations leaving the surrounding skin intact.  These laser systems are able to fragment the target into small particles making them easily processed by the human body with the phagocytosis process.

What is the Photo-acoustic Effect?

The Photo-acoustic Effect is when the laser radiation from the laser penetrates the biological tissue (skin) and reaches the destination particle (ink or pigmentation).

Provided the laser has the correct wavelength – the absorption point of the particle will rapidly rise in temperature and this, combined with a short laser pulse will mean the rapid and local warming of the particle at the absorption point will cause a violent expansion – resulting in an Acoustic Shock Wave.

To the Client receiving the treatment this will mean a very loud snapping noise will be heard.  However, on Quanta Lasers ALL of the energy will be in the Acoustic Wave – with NO thermal effect!

What is the Phagocytosis process?

If the laser pulse has been short and strong enough, this Acoustic Shock wave should be enough to fragment the particle.

This particle is then attacked by the macrophages in the body and when small enough to be engulfed by the cell membrane they are digested by the body’s natural immune system.  Successfully removing it through the lymphatic system and out of the body altogether.

What makes the Quanta Q-Switch System different to other Q-Switch lasers?

The Quanta system is a short pulsed laser with the technological breakthrough of OPTIBEAM TECHNOLOGY – Optimal Laser Beam Profile.

The Optibeam handpiece generates a perfect ‘Flat Top’ beam profile with Optical Technology which is able to distribute the power of the laser homogeneously on ALL spot sizes!

This is crucially important technology with regards to Q-Switch lasers as it is this even consistency of power distribution which promotes a healthy and shorter healing time and less negative side effects (burning/blistering/hypo-pigmentation).

The OptiBeam II handpieces

–  can generate different flat-top spot shapes at various sizes.

–  Square flat-top handpieces are ideal for tattoo removal

–  Round holographic handpieces are designed to be adjusted to            pigmented lesions’ shapes

– Fractional handpiece is perfect for skin resurfacing and rejuvenation with very short recovery time.

1) Tattoo Removal & Pigmentation

Do you have unwanted marks on your skin?

Whether these be Tattoos or pigmentation conditions – you are not alone – 1 in 2 people these days across the world have such marks or imperfections in their skin that they would like to remove.

Laser technology represents nowadays the most effective, safe and non-invasive way to eliminate these imperfections.

For Pigmentation this treatment is indicated to patients with benign skin spots like lentigo, freckles, melasma, post-pregnancy spots, post inflammatory hyper-pigmentation, Ito and Ota nevi.

If not clearly benign, we recommend Medical examination of these disorders before embarking on laser treatment.

What is the laser removal of Benign Pigmented Lesions?

Senile and solar benign pigmented lesions can appear with skin ageing, especially on the body areas that have been exposed to the sun the most.

Other kind of pigmented lesions can have different origins and can even be present since birth, after skin injury or after pregnancy.

The laser technology is capable to emit extremely short pulses which can selectively interact with undesired melanin concentrations of the skin spots removing them quickly and effectively.

How many treatments will I need?

This varies person by person.  Amateur tattoos can usually be removed in 3-4 treatments with professional tattoos taking 6-12 treatments.  Whilst skin pigmentation treatments can take anything from 3-8 sessions.

Does it hurt?

Tattoo removal treatment and skin pigmentation treatments are relatively painless.

The Photo-acoustic effect of the laser is often the worst part!  The noise makes many Clients jump in anticipation of pain – even though there isn’t really any!

To ensure client comfort we use a “cooler” machine which blows cold air onto the area being treated to ensure little discomfort is felt.

How often will I need to come for treatment?

Every 6 – 8 weeks or until the skin is completely healed.

No deodorant, perfumes or scented body lotions/creams

No swimming or Jacuzzis

Do not scratch or rub the treated area

Do not wear tight or restrictive clothes over the area treated for 48 hours

How should I prepare for treatment?

Do not use products containing retinol or strong AHA fruit acids prior to or during your tattoo removal treatment

Avoid swimming in strong chlorinated water immediately before treatment

Avoid wearing tight clothing when attending the treatment

Physical sun cream should be worn at all times on areas visible to sunlight (SPF 30)

Avoid sun exposure and sun beds for 4 weeks before treatment

Do not use fake tanning products for two weeks prior to treatments including spray tanning and tinted moisturisers

If the tattoo/pigmentation is in a hairy area please shave the day before treatment

How will my tattoo look directly after treatment?

Immediately after tattoo removal treatment your tattoo will look red, raised, swollen with white frosting where the pigment has been shattered.  This can last up to a week and can be followed by bruising.

It is extremely important that you follow the after-care advice given by your practitioner.

Quanta Q-Plus A – Non-Ablative Fractional Skin Resurfacing

Without a doubt one of the most astounding features of our Quanta Q-switch system is the Non-Ablative Fractional handpiece which resurfaces and rejuvenates skin like no other laser we have used before!

Fractional laser is one of the advanced techniques, which is frequently used for skin rejuvenation. Non-ablative fractional laser resurfacing is developing very rapidly to become the gold standard for laser resurfacing therapy for various indications including photo rejuvenation therapy and acne scarring reduction.

The system is so versatile, it literally provides the answer to all of our client’s skin concerns – including hyper-pigmentation, acne scars, dullness, fine lines – and rejuvenates the skin in a way that reveals a new layer of glowing, healthy skin.

When someone has suffered with their skin throughout their life it can become soul destroying trying multiple methods and treatments that all promise to ‘fix’ long term skin concerns – particularly in the cases of so many of our clients who suffer from acne scarring.  Yet finally we have found a solution that is giving us unrivalled results on damaged skin which in our 12 established years we have never seen before!…  and the best bit is that as the laser is non-ablative there is very little ‘down time’ and the results become visible after just one session.

Which is why this on-the-rise treatment has become a solution for people serious about blasting away imperfections for good.

Without a doubt, nothing gives us more satisfaction that seeing the rush of self esteem it gives acne scarring clients.  Equally pleasing is witnessing the results it has on photo-damaged and ageing skin – fixing skin quality and density issues that injectable treatments like Botox© and dermal filler alone cannot.

For textured acne scars in particular (think: ice pick, boxcar, and rolling scars) we are seeing greater results than we have with any other treatment method or laser (and this includes C02 – we no longer provide this treatment as we find this laser far superior)

The non-ablative handpiece on the machine can treat hyper-pigmentation or sun damage, wrinkles, and acne scarring.  Clients most commonly have lasers applied to their whole faces, but the neck, décolletage, hands and arms are also popular areas to treat.

What is a Non-Ablative Fractional Laser?

Non-ablative skin rejuvenation uses a Fractionated handpiece on the laser to improve the appearance of wrinkles, brown spots and minor scars by creating heat in the skin without injuring the surface of the skin.

The Quanta Fractional handpiece works without injuring the surface of the skin.

With fractional laser resurfacing, the laser beam is broken up or fractionated into many small micro beams that are separated so that, when they hit the skin surface, small areas of the skin between the beams are not hit by the laser and are then left intact.

The heat that is delivered into the skin is done so through thousands of tiny, deep columns known as ‘microthermal treatment zones’, with intervening, normal untreated skin,

These small areas of untreated skin promote much more rapid recovery and healing, with less risk of complications.  The microthermal treatment zones cause sufficient laser injury deep in the dermis to promote new collagen production and resultant facial skin rejuvenation.

The heat generated by the laser promotes collagen production which causes the skin to tighten and look young and healthy.

The fractional approach allows the skin to heal much faster than if the entire area was treated.

This approach lessens the recovery period and reduces the number of complications that can occur.  Multiple sessions are needed in most cases to achieve the final results – but the results are absolutely outstanding and far excel the results we as a Clinic have seen from ‘one off’ ablative treatments trying to achieve the same thing but with horrendous down time and half the results.

Who is a Suitable Candidate?

This treatment is advisable for any skin type with ageing signs or for anyone with scarring – acne or otherwise.

“Hectic lifestyle and the natural ageing process, especially without adequate precautions such as avoiding smoking and excessive sun exposure, could cause premature skin ageing. This condition usually leads to the appearance of wrinkles, spots and facial vascular lesions as a result.  Nowadays it is possible, through laser treatments, to stimulate the skin to regenerate itself without using medicines.  The rediscovery of a luminous and toned face is now finally a real possibility.”

Is there any downtime? How many sessions will I need?

The fact that this fractional laser is ‘non-ablative’ is the best bit.  Downtime is absolutely minimal and depending on skin type, settings and protocol, most clients are recovered in 72 hours – with make up being able to cover any evidence of the treatment pretty much immediately (the next day).

It is possible to notice skin redness and some swelling in the treated area immediately after the session.  This condition will disappear in few days.  It is also possible to notice a temporary heat sensation and even small crusts.

The treatment need not interfere with everyday life in any way.

It is possible to obtain great results even with just one or two sessions, the number may vary according to the kind and the entity of the skin condition.  We certainly have with every treatment we have carried out so far!

It will be possible to observe a smoother skin, in case of anti-wrinkle treatment, and an improvement in the skin texture in case of scars, after just a couple of weeks.

The final results will be gained a few months after the last session.
